The following questions are designed to give me an overview of you, your relationship history and current concerns. Some questions you may feel more comfortable discussing in session — please just note that in the form and we can explore it together.

Where I am working with a couple, your form is confidential to you and me. Please be aware that I am unable to hold secrets — if one partner shares a significant secret their partner is unaware of and does not intend to disclose, I would need to refer you both to a new therapist to ensure the work remains ethical and safe.
